Keywords: Custom Titanium Mesh, Guided Bone Regeneration, Dental Implant Objectives: Reconstruction of alveolar bone defects represents an important step in oral surgery prior to implant-supported rehabilitation. Custom titanium mesh systems used for guided bone regeneration (GBR) have become increasingly popular in recent years due to their advantages in three-dimensional planning and mechanical stability. The aim of this study was to evaluate the clinical and radiological outcomes of seven patients treated with custom titanium mesh. Materials and Methods: Seven patients who underwent bone augmentation with custom titanium mesh due to insufficient alveolar bone volume were included in this study. Digital planning was performed using cone-beam computed tomography (CBCT) images, and patient-specific titanium meshes were fabricated according to the defect morphology. During surgery, augmentation was performed using particulate autogenous bone grafts, and the meshes were stabilized with titanium screws. Patients were evaluated postoperatively for clinical complications and radiological bone gain. The mean follow-up period was 6 months. Results: Among the seven patients, augmentation procedures were performed in the posterior maxilla in four patients, while one patient each underwent augmentation in the anterior maxilla, total maxillary alveolus, and total mandibular alveolus. The mean vertical bone gain was 3.8 ± 1.1 mm, and the mean horizontal bone gain was 3.3 ± 0.9 mm. Limited mesh exposure occurred in two patients and was managed conservatively without signs of infection. Adequate bone volume for implant placement was achieved in six patients; in one patient, the mesh was removed due to exposure associated with infection. Conclusion: Compared with conventional titanium mesh systems, custom titanium meshes provide improved adaptation and stability, allowing more predictable bone regeneration. Consistent with previous reports, mesh exposure was the most common complication in this case series; however, limited exposures did not negatively affect the overall augmentation outcome. Custom titanium mesh appears to be a reliable and effective technique for the reconstruction of complex alveolar defects.
