ANTERIOR TIBIAL ARTERY PSEUDOANEURYSM FOLLOWING PROXIMAL TIBIAL FRACTURE SURGERY: SUCCESSFUL TREATMENT WITH COIL EMBOLIZATION

Pseudoaneurysm formation following orthopedic surgery is a rare but potentially serious vascular complication. The anterior tibial artery is particularly vulnerable to iatrogenic injury during proximal tibial fracture fixation due to its anatomical proximity to the surgical field. A 63-year-old female patient with hypertension presented with pain and a pulsatile mass approximately two months after proximal tibial fracture surgery, located 10 cm inferior to the left knee joint. CTA demonstrated a large pseudoaneurysm measuring approximately 15 cm in diameter originating from the anterior tibial artery. The patient was referred to Interventional Radiology, where coil embolization at the pseudoaneurysm neck was successfully performed. The pulsatile mass became non-pulsatile immediately following the procedure, and control imaging confirmed complete thrombosis of the sac with no residual flow. No signs of distal limb ischemia were observed. CTA is the diagnostic modality of choice for peripheral pseudoaneurysms, and endovascular coil embolization is a safe and effective minimally invasive treatment. A multidisciplinary approach involving orthopedic surgery, cardiovascular surgery, and interventional radiology is essential for optimal outcomes.
