Abstract

Objective:
Internal root resorption (IRR) is a rare type of root resorption characterized by dentin loss associated with odontoclastic activity secondary to an inflammatory response within the pulp tissue. Due to its often asymptomatic nature, it may be incidentally detected during routine radiographic examinations. This case report aims to emphasize the importance of careful radiographic evaluation in the early diagnosis of IRR and to highlight its contribution to accurate diagnosis and appropriate treatment planning.

Materials and Methods:
A routine panoramic radiograph was obtained from a 21-year-old systemically healthy patient who presented to our clinic with pain in the third molar region. Panoramic radiographic examination revealed suspected root resorption associated with the maxillary right lateral incisor, and a periapical radiograph was subsequently obtained for further evaluation. Periapical radiography demonstrated irregularity in the canal contour and loss of continuity of the canal lumen at the coronal third of the root. Cone-beam computed tomography (CBCT) examination was performed for detailed assessment of the resorptive lesion.

Results:
CBCT evaluation revealed a resorptive defect associated with the pulp canal space and extending toward the buccal aspect at the coronal third of the root of the maxillary right lateral incisor. Preservation of the external root contour and the continuity between the resorptive area and the canal space were consistent with internal root resorption. The patient was referred to the Department of Endodontics for treatment planning.

Conclusion:
This case highlights the importance of careful radiographic evaluation in the early diagnosis of internal root resorption and demonstrates the value of CBCT in determining the localization and extent of resorptive lesions. Three-dimensional imaging contributes to accurate diagnosis and appropriate treatment planning in cases of internal root resorption.
