Özet

GİRİŞ:Gingival resesyon, kök yüzeyinin açığa çıkmasına bağlı olarak estetik problemlere, dentin hassasiyetine ve plak retansiyonunda artışa neden olabilmektedir. Tünel tekniği, interdental papillaların korunmasına olanak sağlayan minimal invaziv mukogingival cerrahi yaklaşımlardan biridir. Bu olgu sunumunun amacı, maksiller sol kanin diş bölgesindeki Miller Sınıf I gingival resesyonun tünel tekniği ve trombositten zengin plazma (PRP) kullanılarak yeniden koronolde pozisyonlandırılmasının klinik sonuçlarını sunmaktır. OLGU SUNUMU:31 yaşında sistemik olarak sağlıklı kadın hasta, 23 numaralı dişte diş eti çekilmesi şikâyetiyle kliniğimize başvurdu. Klinik muayenede Miller Sınıf I diş eti çekilmesi tespit edildi. Başlangıç periodontal tedavinin ardından lokal anestezi altında tünel tekniği kullanılarak alıcı saha hazırlandı. Hazırlanan tünel içerisine hastadan elde edilen trombositten zengin plazma (PRP) yerleştirildi. Flep, kök yüzeyini tamamen örtecek şekilde koronale pozisyonlandırılarak stabilize edildi. Ardından işlem bölgesi periodontal pat ile kapatıldı. Hastaya postoperatif bakım önerilerinde bulunuldu. SONUÇ:Operasyon sonrası erken dönemde herhangi bir komplikasyon izlenmedi. 14 gün sonra bölgedeki periodontal pat uzaklaştırıldı ve süturlar alındı.1 ay sonra yapılan kontrolde 23 numaralı dişin kök yüzeyinde belirgin kapanma elde edildi. Klinik değerlendirmede sondalama derinliğinde herhangi bir artış saptanmadı ve periodontal dokuların stabilitesinin korunduğu izlendi. Elde edilen klinik sonuçlar estetik açıdan başarılı olarak değerlendirildi. Anahtar Kelimeler: Diş eti çekilmesi; PRP; Tünel tekniği

AIM:Gingival recession is characterized by exposure of the root surface due to apical migration of the gingival margin, which may result in esthetic concerns, dentin hypersensitivity, and increased plaque retention. The tunnel technique is a minimally invasive mucogingival surgical approach that preserves the interdental papillae. This case report aims to present the clinical outcomes of treating a Miller Class I gingival recession in the maxillary left canine region using the tunnel technique and platelet-rich plasma (PRP) followed by coronal repositioning of the gingival tissues. CASE:A 31-year-old systemically healthy female patient presented to our clinic with a complaint of gingival recession affecting tooth 23. Clinical examination revealed a Miller Class I gingival recession. Following initial periodontal therapy, the recipient site was prepared using the tunnel technique under local anesthesia. Platelet-rich plasma (PRP), prepared from the patient's own blood, was placed within the tunnel. The gingival flap was coronally repositioned to completely cover the exposed root surface and stabilized with sutures. The surgical site was subsequently protected with a periodontal dressing. The patient received postoperative instructions and was scheduled for follow-up examinations. CONCLUSION:No postoperative complications were observed during the early healing period. Fourteen days after surgery, the periodontal dressing was removed and the sutures were taken out. At the one-month follow-up, substantial root coverage was achieved on tooth 23. Clinical examination revealed no increase in probing depth, and periodontal tissue stability was maintained throughout the healing period. The clinical outcome was considered esthetically successful.
