Özet

Travma sonrası endodontik tedavi görmüş genç daimi anterior dişlerde geniş koronal madde kaybı ve renk değişikliği, estetik ve fonksiyonel rehabilitasyonu güçleştirebilir. Bu olgu sunumunun amacı, doğal diş dokularını mümkün olduğunca koruyan ve cam fiber ağ ile güçlendirilmiş direkt kompozit yaklaşımın kısa dönem klinik sonucunu sunmaktır. On sekiz yaşındaki erkek hasta, 13 yaşında geçirdiği anterior dental travma sonrasında 2019 yılında kök kanal tedavisi uygulanmış maksiller anterior dişlerindeki geniş koronal doku kaybı ve belirgin renk değişikliği nedeniyle estetik şikâyetle başvurdu. Extensive coronal tissue loss and discoloration in endodontically treated young permanent anterior teeth following trauma may complicate esthetic and functional rehabilitation. This case report aimed to present the short-term clinical outcome of a direct composite approach reinforced with a glass-fiber net while preserving natural dental tissues as much as possible. An 18-year-old male presented with esthetic concerns related to extensive coronal tissue loss and marked discoloration of maxillary anterior teeth that had undergone root canal treatment in 2019 after dental trauma sustained at the age of 13. No active clinical symptoms were detected. Current radiographs were compared with images obtained approximately five years earlier, and no evident changes were observed in the apical or periapical regions. Existing restorations and secondary carious/infected tissues were removed. After tissue removal, the remaining tooth structure showed an acceptable color for esthetic restoration; therefore, no additional bleaching procedure was considered necessary. Considering the patient's young age, preservation of natural tooth structure, and the possibility of future intervention, a minimally invasive 211 treatment strategy was adopted. Under rubber dam isolation, coronal gutta-percha was removed to approximately 3 mm apical to the cementoenamel junction while preserving the apical root filling. A palatal wall was created with direct composite resin. A pre-impregnated glass-fiber net material, everStickNET (GC), was adapted from the coronal root canal space into the direct composite restoration to provide reinforcement. GLUMA Bond Universal was applied using a total-etch (etch-and-rinse) protocol. Esthetic layering was completed with Estelite Sigma Quick (Tokuyama Dental) in A2 and OA2 shades. At the 6-month follow-up, the restorations maintained their functional, biological, and esthetic integrity. This case demonstrates that extensive post-traumatic coronal tissue loss in young patients can be rehabilitated using a conservative, repairable, and tissue-preserving approach.
