Amaç: Keratinize diş eti, serbest diş eti kenarından mukogingival hatta uzanan alan olarak tanımlanır. Diş eti çekilmesi ise diş eti marjininin, mine-sement birleşiminden apikale doğru yer değiştirmesidir. Diş eti çekilmesinin ilerlemesini durdurmak, estetik görünümü düzeltmek ve periodontal sağlığın idamesini kolaylaştırmak için çeşitli mukogingival cerrahi işlemler kullanılmaktadır. Serbest diş eti grefti yapışık diş eti genişliğini artırmak için uygulanan bir prosedürdür. Bu olgu sunumunun amacı 31 nolu dişin bukkalinde serbest diş eti greftiyle yapışık diş eti miktarının arttırılmasının klinik sonuçlarının ve takibinin paylaşılmasıdır. Olgu Sunumu: 17 yaşında sistemik sağlıklı erkek hastanın yapılan ağız içi muayenesinde ortodontik diş hareketine ve yüksek frenulum ataçmanına bağlı olarak 31 nolu dişin bukkalinde yapışık diş etinin yetersiz olduğu ve diş eti çekilmesinin meydana geldiği tespit edildi. Cerrahisiz periodontal tedavisi tamamlandıktan sonra ameliyata alınan hastanın alt çene anterior bölgesinden 41, 31 ve 32 nolu dişleri kapsayacak şekilde yarım kalınlık flep kaldırıldı, kaldırılan flep vestibül sulkusun en derin yerine süture edildi. Damaktan serbest diş eti grefti alınarak alıcı sahaya adapte edilip 5/0 prolen sütur ile sabitlendi. Kanama kontrolü sağlandı. Bölgenin üzeri periodontal pat ile örtüldü. Sonuç: Operasyon sonrası erken dönemde herhangi bir komplikasyon izlenmemiş olup 14 gün sonra bölgedeki periodontal pat uzaklaştırılıp ve süturlar alındı. 1 ay sonra yapılan kontrolde keratinize diş eti genişliği ve vestibuler sulkus derinliğinde artış, açık kök yüzeyinde kapanma olduğu gözlendi. Serbest diş eti grefti operasyonlarının keratinize gingival bandı artırmada iyi bir tedavi yöntemi olabileceği ve var olan diş eti çekilmesinin kapanmasını sağlayabileceği gözlendi. Aim: Keratinized gingiva is defined as the area extending from the free gingival margin to the mucogingival junction. Gingival recession is the apical displacement of the gingival margin from the cementoenamel junction. Various mucogingival surgical procedures are used to halt the progression of gingival recession, improve esthetic appearance, and facilitate the maintenance of periodontal health. Free gingival grafting is a procedure performed to increase the width of attached gingiva. The aim of this case report is to present the clinical outcomes and follow-up of increasing the amount of attached gingiva on the buccal aspect of tooth 31 using a free gingival graft. Case Presentation: Intraoral examination of a 17-year-old systemically healthy male patient revealed insufficient attached gingiva and gingival recession on the buccal side of tooth #31, associated with orthodontic tooth movement and a high frenulum attachment. Following nonsurgical periodontal therapy, the patient underwent surgery. A partial-thickness flap was raised in the lower anterior region, covering teeth 41, 31, and 32, and was sutured at the deepest point of the vestibular sulcus. A free gingival graft was harvested from the palate, adapted to the recipient site, and stabilized with 5-0 prolene sutures. Hemostasis was achieved, and the area was covered with periodontal dressing. Result: No complications were observed in the early postoperative period. Fourteen days after the operation, the periodontal dressing and sutures were removed. At the one-month follow-up, an increase in the width of keratinized gingiva and vestibular sulcus depth, as well as root surface coverage, were observed. Free gingival graft procedures appear to be an effective treatment option for increasing the keratinized gingival band and achieving root coverage in areas of existing gingival recession.
