ABSTRACT:
Objective: Appendix testis torsion is a benign cause of acute scrotum that may clinically mimic
testicular torsion and create diagnostic challenges. This study aimed to present our clinical experience
in the diagnosis and management of appendix testis torsion. Methods: Three pediatric patients
presenting with acute scrotal pain and diagnosed with appendix testis torsion were retrospectively
evaluated. Clinical findings, laboratory results, Doppler ultrasonography findings, treatment
approaches, and clinical outcomes were analyzed. Results: The patients were 9, 12, and 14 years old
and presented with sudden onset unilateral testicular pain. Physical examination revealed testicular
tenderness in all cases, hile the blue dot sign as observed in one patient. olor Doppler
ultrasonography demonstrated preserved testicular vascularization and avascular lesions compatible
with torsion of the appendix testis in all patients. Two patients were successfully managed with
conservative treatment, including analgesics, scrotal elevation, and rest, with complete resolution of
symptoms. One patient required surgical exploration due to persistent severe pain, and a necrotic
appendix testis was excised. All patients had favorable outcomes without complications. Conclusion:
Appendix testis torsion should be considered in the differential diagnosis of acute scrotum in children.
Careful clinical evaluation and Doppler ultrasonography are essential for accurate diagnosis, allowing
conservative management in most cases while reserving surgical exploration for selected patients.
