Amaç: Yapışık diş eti, periodontal dokuların biyolojik bütünlüğünü korumada önemli rol
oynamaktadır. Vestibüler sulkus derinliği ve keratinize doku genişliğinin yetersiz olduğu bireylerde
periodontal sağlığın sürdürülmesi zorlaşmaktadır. Serbest diş eti grefti tedavisiyle; keratinize diş
eti miktarının artırılması ve uzun dönemli periodontal sağlığın desteklenmesi hedeflenmektedir. Bu
olgu sunumunun amacı, 31 numaralı dişin vestibül yüzeyine uygulanan serbest diş eti greftinin
klinik sonuçlarını ve tedavi sonrası iyileşme sürecini değerlendirmektir.
Olgu Sunumu: 17 yaşında, sistemik olarak sağlıklı erkek hastanın klinik muayenesinde, 31
numaralı dişin vestibül yüzeyinde yetersiz yapışık diş eti ve diş eti çekilmesi olduğu saptandı.
Cerrahisiz periodontal tedavi tamamlandıktan sonra serbest diş eti cerrahisi planlandı. Cerrahi
esnasında 32, 31 ve 41 numaralı dişleri kapsayan yarım kalınlık flep kaldırıldı ve vestibüler
sulkusun en derin noktasına 4.0 rezorbe sütur ile dikildi. Sert damaktan alınan serbest diş eti grefti
alıcı bölgeye adapte edilerek 5.0 prolen ile sabitlendi. Kanama kontrolünün ardından bölge
periodontal pat ile örtüldü. Hastaya postoperatif bakım önerileri verildi.
Sonuç: Postoperatif kontrollerde greftin iyileşmesinde herhangi bir komplikasyon gözlenmedi.
Yaklaşık 4 mm yapışık diş eti elde edildi. Vestibüler derinlikte artış gözlendi. Bu olguda serbest diş
eti grefti ile başarılı bir klinik sonuç elde edildi. Serbest diş eti greftlerinin yapışık diş eti miktarı
yetersizliği tedavisinde kullanımının, keratinize doku genişliğini ve vestibüler sulkus derinliğini
artırmak amacıyla etkili ve güvenilir bir tedavi seçeneği olduğu düşünülmektedir.

Aim: Attached gingiva plays an important role in maintaining the biological integrity of periodontal
tissues. Maintaining periodontal health is difficult in individuals with insufficient vestibular sulcus
depth and keratinised tissue width. The aim of free gingival graft treatment is to increase the
amount of keratinised gingiva and support long-term periodontal health. The purpose of this case
presentation is to evaluate the clinical results and post-treatment healing process of a free gingival
graft applied to the vestibular surface of tooth number 31.
Case Presentation: A 17-year-old male patient in good systemic health was found to have
insufficient attached gingiva and gingival recession on the vestibular surface of tooth number 31
during clinical examination. After completing non-surgical periodontal treatment, free gingival
surgery was planned.During surgery, a half-thickness flap covering teeth numbers 32, 31, and 41
was raised and sutured to the deepest point of the vestibular sulcus with 4.0 resorbable sutures. A
free gingival graft taken from the hard palate was adapted to the recipient site and secured with 5.0
Prolene sutures. After bleeding control, the area was covered with periodontal pat. The patient was
given postoperative care instructions.
Result: No complications were observed in the healing of the graft during postoperative check-ups.
Approximately 4 mm of attached gingiva was obtained. An increase in vestibular depth was
observed. In this case, a successful clinical outcome was achieved with a free gingival graft. The use
of free gingival grafts in the treatment of insufficient attached gingiva is considered to be an
effective and reliable treatment option for increasing the width of keratinised tissue and the depth
of the vestibular sulcus.
